The fax machine sitting in the corner of your doctor's office isn't a museum piece. In real terms, it's still working. Every day.
You've probably heard someone declare the fax dead at least a dozen times. Maybe you've said it yourself. But walk into a law firm, a hospital, a government agency, or a real estate office and you'll hear that familiar handshake tone — the screech and hiss of two machines agreeing to talk. It's not nostalgia. It's infrastructure.
What Is a Fax Machine
At its core, a fax machine is a scanner, a modem, and a printer rolled into one box. Also, it takes a physical document, converts it into audio signals, sends those signals over a standard telephone line, and reconstructs the document on the other end. The name comes from "facsimile" — Latin for "make alike.
The first commercial fax service launched in 1865, decades before the telephone. It used pendulums and chemically treated paper. Now, by the 1980s, thermal paper rolls curled out of machines in offices worldwide. In real terms, then came plain-paper laser and inkjet models. The technology evolved, but the fundamental job never changed: move a document from point A to point B without putting it in an envelope.
The Three Main Types Still Around
Standalone thermal or laser fax machines — the classic box with a handset, keypad, and paper tray. You feed pages face down, dial a number, press Start. They're simple, reliable, and still sold new today.
Multifunction printers with fax built in — most office MFPs now include a fax modem as an optional or standard feature. One device prints, scans, copies, and faxes. Saves space. Adds a single point of failure.
Fax servers and cloud fax services — no physical machine required. Incoming faxes arrive as PDFs in email or a web portal. Outgoing faxes go from a print driver or web upload. The telephone line connects to a gateway appliance or lives entirely in the cloud. This is how most enterprises handle volume now.
Why It Matters / Why People Still Care
Email is faster. Scanning is easier. Cloud storage is cheaper. So why does fax refuse to die?
Legal and Regulatory Weight
In many jurisdictions, a faxed signature carries the same legal weight as an original ink signature. S. HIPAA considers fax a compliant transmission method for protected health information when proper safeguards exist. ESIGN Act and UETA explicitly include fax. But courts accept faxed filings. The "Original" Problem
A scanned PDF can be edited, compressed, re-saved, and forwarded without a trace. A fax transmission creates a timestamped, point-to-point record. Try getting that level of audit trail from an email attachment without extra software.
Network Effects in Regulated Industries
Healthcare is the big one. Switching an entire industry to a new standard takes coordination, budget, and a compelling reason. Referrals, prior authorizations, lab orders, discharge summaries — massive volumes move by fax every day. Electronic health record systems often integrate directly with fax servers. "Fax works" is a compelling reason to stay put.
No Internet Required
A fax machine needs a phone line and electricity. That's it. In rural clinics, disaster zones, secure facilities with air-gapped networks, or during internet outages, fax keeps working. On the flip side, it's not a feature. It's a fallback.
How It Works
The process hasn't changed much since Group 3 fax became the standard in 1980. Here's what actually happens when you press Start.
Scanning and Encoding
The document feeds past a CCD or CIS sensor — the same tech in a flatbed scanner. Practically speaking, light reflects off the page. The sensor captures a line of pixels at a time, typically 203 dpi horizontally and 98 or 196 dpi vertically (standard vs. Still, fine resolution). Each pixel becomes a 1 or 0: black or white.
Those bits get compressed using Modified Huffman (MH) or Modified READ (MR) coding. Run-length encoding counts consecutive white or black pixels instead of storing each one. Because of that, a blank line compresses to almost nothing. A dense page of text compresses less. The result is a bitstream ready for the phone line.
The Handshake
Your machine picks up the line and sends a CED tone — 2100 Hz — to announce "I'm a fax." The receiving machine answers with a DIS signal listing its capabilities: max speed, supported compression, paper sizes, etc. Your machine replies with DCS, selecting the highest common settings. Then both sides train their equalizers using a test pattern to compensate for line noise.
You hear all of this. The screech is the training sequence. The warbling is data modulated on the carrier.
Modulation and Transmission
Group 3 fax uses V.27ter (2400/4800 bps), V.29 (7200/9600 bps), V.17 (7200/9600/12000/14400 bps), or V.But 34 (up to 33600 bps) modulation. The machine picks the fastest speed the line quality supports. Data flows in frames with error correction (ECM — Error Correction Mode) retransmitting bad frames automatically.

A typical letter page at 9600 bps with compression takes 30–60 seconds. At 14400 bps with ECM, maybe 20 seconds. Color fax exists (ITU-T T.30e) but almost no one uses it.
Reception and Printing
The receiving machine decodes the bitstream, decompresses it, and sends raster lines to its print engine. Laser and inkjet models render to a drum or fire nozzles. Here's where it gets messy. It works — when both gateways support T.38, the network has low jitter, and the firewall plays nice. Fax over IP (T.VoIP lines compress voice with lossy codecs (G.In practice, 38) solves this by demodulating the fax signal at the gateway, sending the raw bitstream as UDP packets, and remodulating at the far end. Common Mistakes / What Most People Get Wrong
Assuming Any Phone Line Works
Plug a fax machine into a digital PBX extension, a cable modem's phone port, or a VoIP adapter without T.Day to day, 38 support and you'll get failed transmissions, missing pages, or garbage output. Analog lines (POTS) are the gold standard. If you must use VoIP, verify T.
support end-to-end and configure your ATA for fax mode (disable VAD, silence suppression, and echo cancellation). Even then, marginal lines cause fallback to 2400 bps or total failure.
Ignoring ECM
Error Correction Mode adds overhead but saves transmissions on noisy lines. Some administrators disable it to "speed things up." On anything but pristine copper, this backfires — a single corrupted frame forces a full-page retransmission instead of a partial frame retry. Leave ECM on.
Misconfiguring Resolution
"Fine" (200×196 dpi) and "Super Fine" (200×391 dpi) modes quadruple data versus "Standard" (204×98 dpi). Still, most documents don't need it. That said, handwritten notes, signatures, or halftone photos benefit; plain text doesn't. Default to Standard unless you have a reason not to.
Forgetting the Confirmation Page
A transmission report with "OK" and a page count is your proof of delivery. No report? You have no evidence the fax arrived intact. Configure machines to print confirmations automatically and archive them for anything legally sensitive.
Treating Fax as Secure
Standard fax has zero encryption. 38 over SIP/TLS) or VPN tunnels, but most deployments skip this. T.38 over IP can run inside TLS (T.Anyone tapping the line — at the demarc, the PBX, or the central office — sees every pixel. For PHI, PII, or classified data, fax is non-compliant without additional controls.
Assuming the Paper Tray Is Full
The number one cause of "fax received but not printed" is an empty tray, a jam from three days ago, or a toner cartridge the machine refuses to acknowledge. Check the physical device before blaming the protocol.
The Uncomfortable Persistence
Fax refuses to die because it solves a coordination problem nothing else has: universal, unmediated, legally recognized document exchange between parties who share no platform, no account, no prior relationship — only a phone number.
Email requires spam filters, DKIM/SPF/DMARC alignment, and mutual deliverability. Portals require accounts, passwords, and compatible browsers. Consider this: blockchain solutions require wallets and technical literacy. Fax requires a machine, a line, and a button.
Healthcare, legal, government, and real estate sectors run on fax not from ignorance but from regulatory inertia and network effects. HIPAA's "conduit exception" treats fax as a pipe, not a business associate — no BAA required. Courts accept fax filings with original signatures. Japanese bureaucracy requires* it.
The infrastructure is decaying. And copper loops are being retired. Central offices convert to packet switching. That's why t. On top of that, 38 gateways are legacy code maintained by shrinking teams. Yet every replacement — eFax, digital signatures, Direct messaging, FHIR APIs — fragments the network further. None achieves the "anyone to anyone" property.
So the screech continues. Somewhere, right now, a thermal printhead heats a line of dots. Because of that, a confirmation page prints. A human tears it off, staples it to a chart, and files it.
The protocol is obsolete. The workflow is not.
