If the person is in water, the first move is to pull them out — but not just onto any surface. In practice, shave off excessive chest hair if the pads won't stick, and wipe away any moisture. Then, before applying the pads, you need to dry the chest thoroughly. Move them to a dry, firm area. This all takes seconds, but it's a step people skip under panic, and skipping it creates real risk.
